Assessment and Treatment with Phoenix

At Phoenix we provide the following range of specialised Occupational Therapy Services:
Functional Capacity Assessments
Phoenix Occupational Therapy perform Functional Capacity Assessments, as one of their main types of assessments for our clients. These are designed to understand the needs of NDIS participants, so that the appropriate supports, therapy services and equipment can be recommended, to enable them to live their life to the fullest. These assessments normally take 10 hours to complete; and are usually completed face-to-face, if possible, in a participant's own home environment. Telehealth appointments can also be arranged if there are challenges to meeting in person.
AT/Equipment Prescription
Phoenix have over 20 years of experience with prescribing a range of equipment from minor adaptive aids such as dressing, toileting and showering aids; to more complex AT such as manual and electric wheelchairs; scooters; electric beds and pressure mattresses; hoists and slings. Should you require an assessment for AT, please get in contact with Phoenix today.
Home Based Therapy Programs
Our therapists are available to visit you in your own home or local community to ensure therapy is accessible, comfortable and available, without the hassle of worrying about transport and having to replicate your familiar surroundings. At home, we can familiarise ourselves with an individual's personal environment, their routines and activities, and implement strategies which are relevant directly to a client's everyday lifestyle and occupation, whilst ensuring the comfort and privacy of our participants.
SIL/SDA Assessments
A SIL assessment is a professional review of your situation, your needs and your goals. Phoenix OT's are capable of completing SIL assessments to provide an extensive evaluation of your current and future support needs, with a high degree of professionalism and attention to detail.
An SDA assessment provides a recommendation to NDIS regarding your eligibility for specialist disability accommodation and the specific environmental features required for optimal independence and safety. This can include specific recommendations around wheelchair accessible homes, installation of ceiling hoists, use of home automation technology or robust features to aid safety.
SIL can be provided in non-SDA residences and SDA residences. This is a detailed service that requires us to provide a comprehensive assessment and report. Therefore, we estimate it will take approximately 25 hours to complete.
Minor Home Modifications
Home modifications are custom-built changes to your home to help you access and use areas of your home. These may be required to improve a client or their carer's safety, accessibility, and to enable the achievement of an individual's goals within their home environment. At Phoenix we have extensive experience with the installation of minor modifications (rails, ramps, flooring solutions, shower accessibility options) and work with trusted suppliers to deliver optimal outcomes for our clients.
Other OT services we provide include:
- Cognitive Assessment and Treatment options
- Upper Limb Rehabilitation
